The City Commission adopts and ratifies the Commencement Notice, Acknowledgment of Receipt of Program Commencement Notice, Information Fact Sheet, and Employee Declination to Participate forms, heretofore prepared by the City Administration, copies of which are attached to Ordinance No. 00-66 as Exhibits "C," "D," "E," and "F" respectively. The City Administration is authorized to modify these forms when modification is necessary to facilitate the objectives of this article or when amendment is required to conform the forms to requirements of state or federal law. In the event of a dispute over the meaning, interpretation, or application of Exhibits "B," "C," "D," "E," and "F," the decision of the City Manager as to the meaning, interpretation, or application shall be controlling.
